Our veterans’ home care services are delivered by our team of highly experienced nurses and support staff, as well as a network of preferred specialist providers. A wide range of nursing, care and therapy services are available to help you maintain your health and safety at home.

What could I be eligible for?

Veterans' Home Care

If you are a Gold Card or White Card holder and need a little bit of practical help around the home or with your personal care, on either a short-term or ongoing basis, the Department of Veteran's Affairs (DVA) Home Care program can assist you to maintain your health, quality of life and ability to continue living at home. This service is not designed specifically to meet complex clinical or high-level care needs but provides more of a helping hand around the house, taking some pressure off where you need it. You can access these services by contacting VHC or asking your GP to refer you.

Eligibility

You may be eligible if you are a veteran, war widow or widower who holds a Gold or White veterans’ health card.

Costs

You will be required to pay a small fee for each day you receive Veterans’ Home Care  services.

What could I be eligible for?

DVA Community Nursing

If you are a veteran of the Australian Defence Forces, or a war widow or widower, Veterans’ Affairs provides a Community Nursing Program with access to community nursing services to meet your assessed clinical and personal care needs. The Community Nursing Program assists you to continue living independently by supporting your health and wellbeing. The aim of this program is to enhance independence and health outcomes by avoiding admission to hospital or residential care. You can access this service by contacting Programmed Care or through a referral from your GP or hospital discharge coordinator.

To be eligible to receive community nursing services, you must be a Gold Card or a White Card holder, and undergo an assessment to determine your need for clinical and/or personal care.

To get started, call our friendly team on 13 10 95 and we will guide you through the process of setting up your services with Programmed Care, including visiting you at home to put together your care plan exactly how you want it and establishing your care team.

It’s easy! Call our friendly team on 13 10 95 and we will guide you through the process of setting up your services with Programmed Care, including working with your former provider to seamlessly manage the transfer of your package and the management of your budget.

We work with you to ensure we find the right support workers to match your requirements and build a consistent care team to limit the amount of new staff entering your home. However, sometimes due to circumstances out of our control, we may need to send a new staff member. We will be sure to notify you if your usual care team is unable to visit you.  

Your dedicated Care Coordinator will work with you to tailor your care plan, ensuring you receive support on the days and times you require. If your needs change, whether it be for a once-off occasion or for ongoing services, please inform your Care Coordinator and they will adjust your schedule accordingly. 

At Programmed Care, we want to make sure everyone receives the support they require. If you cannot afford home care services, please contact your local Programmed Care representative on 13 10 95 to discuss available options.
